  • We utilize a third-party marketing vendor, Reputation Elevation, to collect your contact information as well as other information that may be needed for patient assessment such as insurance and diagnostic information (“Collected Information”).  All Collected Information is forwarded to (ABA provider) through Reputation Elevation for the purpose of providing you with a free consultation by (ABA Provider).  By submitting your information through a form on Facebook or on our website, you hereby agree and voluntarily consent to Reputation Elevation’s receipt Collected Information, and forwarding of your information to (ABA Provider).

 Note: No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties
